申请表格（PCCCI）

For PCCCI Membership Application, please download application form and take note the application requirement as below:

  1. SSM Document copy of Form 9, 24, 44 and 49 (or)

SSM Borang D / Borang E (Kaedah 13) and Maklumat Perniagaan & Pemilik Semasa

  1. Membership fee of RM 2000 (make in cheque payable to Perak Chinese Chamber of Commerce and Industry)
  2. Business name card of applicant
  3. A PCCCI member who propose and sign the application form with company stamp
  4. Another PCCCI member who support and sign the application form with company stamp

有关霹中总的会员申请，请下载申请表格及注意以下的申请条件：

  1. SSM Form 9, 24, 44& 49 的复印本 或

SSM Borang D / Borang E (Kaedah 13) 和 Maklumat Perniagaan & Pemilik Semasa

  1. 入会费用 RM2000 (支票抬头写 Perak Chinese Chamber of Commerce and Industry)
  2. 申请人的商业名片
  3. 一位霹中总会员的介绍人（在表格里签名及盖公司印章）
  4. 一位霹中总会员的赞成人（在表格里签名及盖公司印章）
