1. SSM Document copy of Form 9, 24, 44 and 49
2. Membership fees of RM 2000 (make in cheque payable to Dewan Perniagaan Dan Perindustrian China Perak)
3. Business name card of applicant
4. A PCCCI member who propose and sign the application form with company stamp
5. Another PCCCI member who support and sign the application form with company stamp
